Independent Care Health Plan (iCare) is a Health plan with a Medicare contract. Eligible Medicare beneficiaries with physical, developmental or emotional disabilities must: live in the iCare service area; receive full Medicaid benefits, including assistance with Medicare premiums for Medicare Part A and B; have Medicare Parts A and B (and continue paying the Medicare Part B Premium if not otherwise paid for under Medicaid or by another third party); and, not have End-Stage Renal Disease to qualify. Benefit limitations, co-payments and other restrictions may apply. The Medicare Part D Prescription Drug benefit is only available to members of the iCare Medicare Plan. If you are already enrolled in a Medicare Advantage Prescription Drug Plan, you must receive your Medicare Prescription Drug benefit through that plan until your iCare Medicare Plan coverage begins. Members must use network pharmacies and plan providers for routine care. If you obtain routine care from out-of-plan providers neither Medicare nor iCare will be responsible for the costs. iCare’s contract with the Centers for Medicare and Medicaid Services (CMS) is renewed on a yearly basis and benefit coverage beyond the end of the current contract year is not guaranteed. Benefits, formulary, pharmacy, network premium and/or copayments/co-insurance may change on January 1, 2008. Please contact iCare for details. Documents can be made available in alternative formats such as Braille, certain foreign languages, audio tapes and large print.
