Benefits Services
Our Member Services Department is your link to everything you want to
know about iCare. If you need information quickly and cannot reach
your Care Coordinator, call Member Services at 414-223-4847.
Call Member Services each time you need transportation to a doctor's
appointment. We provide rides by cab, bus or special motorized vehicle (SMV
- requires your doctor's approval)
to your medical appointments.
Transportation
Rides to your medical appointments
Bus, taxi or SMV rides to receive care are
arranged by iCare
for members in Milwaukee county with
iCare Medicaid. Call Member Services at 414-223-4847
and press Option#9 to arrange
your ride. You must call for a ride one business day before your appointment.
Ambulance Services
Independent Care covers ambulance service for emergency care. We
may also cover this service at other times, but you must have approval for
all non-emergency ambulance trips. Call our Member Services Department for
approval.
Special Motorized Vehicle (SMV) Services
Independent Care covers transportation by SMV for individuals
who use wheelchairs. Talk to your doctor or call our Member Services
Department three (3) business day in advance if you need SMV service.
Interpreter Services
Independent Care provides you with access to interpreter services. Call
414-223-4847 and press Option #9, or call your Care Coordinator to
schedule an interpreter for your medical appointment three (3) business days
before your appointment.
Interpreter services for the hearing
impaired are provided by
Wisconsin Relay for Hearing Impaired.

Medical Services Covered by
iCare
Independent Care covers all medically necessary services.
Some services may require a doctor's order or a prior
authorization. Covered services include:
- If you have Medicaid, prescription drug coverage
is handled by the State, not iCare.
You are responsible for the co-payments
- If you also have
iCare Medicare
your prescription drugs are
covered by the Medicare Part D benefit
- Services by
iCare
contracted doctors and nurses, including nurse practitioners and
nurse midwives
- Inpatient and outpatient hospital services
- Laboratory and X-ray services
- HealthCheck for members under 21 years of age, including referral
for other medically necessary services
- Certain podiatry (foot doctor) services
- Inpatient care for behavioral health
- Optometrists' (eye doctors) or opticians' services, including
eyeglasses
- Mental health services
- Substance abuse (drug and alcohol) services
- Family planning services and supplies
The following services are covered by
iCare when a doctor gives a written
order:
- Prostheses and other corrective support devices
- Hearing aids and other hearing services
- Home health care
- Personal care
- Independent nursing services
- Medical supplies and equipment
- Occupational therapy
- Physical therapy
- Speech therapy
- Respiratory therapy
- Nursing home services or hospice care
- Medical nutrition counseling
- Transportation by ambulance or specialized motorized vehicles when
needed
- Certain dental services
- Hospice care
Medicine
If you
also have
Medicare, under the new Medicare Part D drug benefit you will be required
to pay a co-pay and follow the rules of your Prescription Drug
Provider (PDP). You can get your prescriptions filled at any pharmacy that works with
iCare.
Just go to one of the drug stores listed in the
iCare
Provider
Directory, show your Forward card and tell them you are an
iCare
member. If you have questions about the pharmacies in our network, you
may call 1-800-788-2949. For any other pharmacy questions, please call
iCare
Pharmacy Services at 414-231-1074.
Those members with Medicare will be required to fill their prescriptions at
their Prescription Drug Plan's approved pharmacies, under the Medicare Part D drug benefit.
