How to Join
What is iCare?
Independent Care is a
Medicaid HMO that manages access to health care and social services for people
with disabilities (physical, developmental and emotional) ages 19 and older.
With
iCare,
we offer the same benefits as Medicaid fee-for-service, or Title 19, but
with more services, which you can find in our
Member Services
section.
Eligibility
If you are at least 19 years of age, live in Milwaukee County
(more counties will be added in 2008), and receive
Medicaid and Supplemental Security Income (SSI) or SSI related benefits, you
are eligible for the program. Medicare beneficiaries or those enrolled in a
Medicare HMO are also eligible, as long as you receive Medicaid or SSI
benefits.
What Are The Benefits?
We cover all the same medical services as
covered by Medicaid. Our care management benefits make us different.
Visits to your doctor and dentist, emergency care, hospital services, mental
health and substance abuse treatment, medical equipment and other services
are covered at 100 percent.
Please note that benefits and services
offered are subject to change.
