What to do When You Join
iCare
When you become a member of
iCare, we will send you a "new
member letter." After you have received your "new member
letter" from us, we would like you to do the following:
- Confirm your phone number and address. Let us know if
your phone number and address is correct by filling out the form on the
back of the member letter and mailing it to us in the enclosed reply
envelope. We will be glad to help you complete the form if you
call us at (414) 223-4847 or 1-800-777-4376 (TTY: 1-800-947-3529 or
7-1-1, Voice: 1-800-947-6644
or 7-1-1).
- Complete your assessment. A
Care
Coordinator
will call you to find out your health care needs. It is very
important that you complete this assessment to help us get you the care
that you need. If you do not receive a call from a Care
Coordinator, please feel free to call us on your own.
- Choose your Primary Care Provider (PCP). After you have
completed your assessment, we will send you a new member kit. The
member kit will include a provider directory, where you can choose a PCP. Women also may choose a women’s health specialist (OB/GYN
or nurse midwife) without a referral from your PCP.
Independent Care has a network of PCPs who are aware of
the needs of our members and have a good understanding of their concerns.
All iCare
network doctors are certified by the Medicaid fee-for-service,
or Title 19 programs. These doctors have agreed to see
iCare members
and believe in iCare’s system of care management.
