BadgerCare Plus Program


iCare BadgerCare Plus

Independent Care Health Plan is a Health Maintenance Organization (HMO) for BadgerCare Plus. Once you are enrolled in BadgerCare Plus, you will receive a packet in the mail that gives you information about the HMOs available to you in your county.

BadgerCare Plus is a State/Federal program that provides health coverage for Wisconsin families. It replaces the former AFDC-Medicaid, Healthy Start and BadgerCare programs. BadgerCare Plus has three major health care benefit plans: Standard, Benchmark and Core.

The Standard Plan is for families with income at or below 200% of the Federal Poverty Level (FPL). Standard plan members may be asked to pay a share of the cost of services. The co-pay amount ranges from $.50 - $3.00 per service.

The Benchmark Plan, which provides more limited services than the Standard Plan, is for families with income above 200% of the FPL, and for self-employed parents and caretakers. Benchmark Plan members will be asked to pay a share of the cost of services provided to them.

The Core Plan for childless adults is an expansion of the BadgerCare Plus program that allows health care coverage for adults who do not qualify for the other BadgerCare Plus plans or Medicaid. The Core Plan covers basic health care services including primary and preventive care, generic and a limited number of brand name prescription drugs.

How do I apply for BadgerCare Plus?

Applicants must submit a signed and completed application form through the State of Wisconsin Department of Health Services to complete the BadgerCare Plus application process. You may fill out an online application at access.wisconsin.gov, schedule a face-to-face interview at your local county/tribal office, mail-in the application form or complete a telephone interview. For questions about the application, contact your local agency, call the State’s Member Services hotline at 1-800-362-3002, or log on to badgercareplus.org.

Who is eligible for iCare BadgerCare Plus?

The iCare BadgerCare Plus plan is available for children under age 19, pregnant women, parents and caretakers of children under 19, young adults leaving foster care when they turn 18, parents with children in foster care, farm families, self-employed families and families at lower-income levels who do not have health insurance. In addition, some childless adults with low income or no access to employee-sponsored health insurance may qualify under the Core Plan.

Our service area is Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Washington and Waukesha counties.

Services Provided by iCare BadgerCare Plus

Mental Health & Substance Abuse

Independent Care provides mental health and substance abuse (drug and alcohol) services to all members.

Tobacco Cessation Services

Tobacco cessation services are provided as office visits and outpatient substance abuse and mental health services.

Family Planning Services

Independent Care provides private family planning services to all of our members. This includes minors. We will help you choose an iCare family planning doctor who may be different from your primary care doctor.

We encourage members to receive family planning services from an iCare doctor. That way we can better coordinate all your health care. Federal law allows members to choose their provider, including doctors and family planning clinics, for reproductive care and supplies. Under iCare BadgerCare Plus, you will be able to go to any family planning clinic that will accept your ForwardHealth ID card even if the clinic is not part of iCare.

Dental Services

Independent Care provides all dental services covered by Medicaid in Kenosha, Milwaukee, Racine and Waukesha counties, but members must go to an iCare dentist.

Members who live in Ozaukee, Sheboygan or Waukesha counties will still receive dental coverage under iCare BadgerCare Plus. However, your dental services will be provided by the State, not iCare.

Chiropractic Services

Members may get chiropractic services from any chiropractor who will accept your ForwardHealth card. Your chiropractic services will be provided by the State, not iCare. When you use these services, you may need to pay a co-pay.

HealthCheck

HealthCheck is a preventive health checkup program for iCare BadgerCare Plus members under the age of 21 (not covered for BadgerCare Plus Core Plan members.) The HealthCheck program includes complete health checkups. The doctor wants to see those under age 21 for regular checkups, not just when they are sick.

The HealthCheck program covers the medical care for health problems found during the checkup including medical care, eye care and dental care. The HealthCheck checkup includes: health and developmental history (including anticipatory guidance), an unclothed physical exam, vision screening, hearing screening, dental screening and a referral to a dentist (beginning at age three), shots appropriate for age and blood and urine lab tests (including blood lead level testing when appropriate for age).

Transportation

If you are a member of the iCare BadgerCare Plus Standard Plan and you live in Milwaukee county, iCare will arrange bus or taxi rides for you to receive health care. If you are a member of the Standard Plan and live in Kenosha, Ozaukee, Racine, Sheboygan, Washington or Waukesha counties, please call your county Department of Social or Human Services for information about arranging a ride.

Non-emergency transportation is not a covered benefit if you are a member of the BadgerCare Plus Benchmark or Core Plan.

Special Medical Vehicle (SMV)

Independent Care covers transportation by special medical vehicle for BadgerCare Plus Standard members in wheelchairs. We may also cover this service for others if your doctor asks for it.

Non-emergency transportation is not a covered benefit for members of Benchmark or Core Plans.

Ambulance

Independent Care covers ambulance service for emergency care for all counties in our service area. We may also cover this service at other times, but members must have approval for all non-emergency ambulance trips.

Pharmacy Benefits

Member prescriptions and certain over-the-counter items are provided by the State, not iCare.

BadgerCare Plus members may receive prescriptions from iCare doctors, specialists or dentists. You can fill your prescription at any pharmacy that is a provider for BadgerCare Plus.

Simply show your ForwardHealth ID card to the pharmacy when you get your prescriptions filled. You may have co-payments or have limits on covered medications.
