Your sister has special needs and your parents are worried about their ability to take care of her. They want to stay involved in her life, but need help. You know that they can’t do this alone … not forever.

iCare: We build a plan of care at iCare for each member that is tailored to their needs. This plan is more the member’s plan than iCare’s plan. Ideally, family members are a key part of the plan and play a role in the success of the plan. Your parents won’t need to do this alone. iCare will share the load.

Our Member Services department is your link to everything you want to know about iCare. Interpreter and translation services are available free of charge.

After-Hours Service

If your condition is severe, go to the nearest emergency room (ER). If you are unable to get to the ER, call 911. For after-hours urgent or emergent care inquiries, please call us at 1-800-777-4376 (TTY 1-800-947-3529) or 414-223-4847, and follow the prompts to get you in contact with a member of the Partnership care team.

Partnership Benefits

The iCare Partnership program covers Medicare, Wisconsin Medicaid, and home and community-based long-term care (LTC) services. Your benefits would include health and medical services, prescription drugs and LTC services. Most services have no out-of-pocket costs except prescription drugs, where you may be responsible for a co-pay if you are on Medicare. The following benefit information is a brief summary, not a comprehensive description of benefits available through the iCare Partnership program:

Emergency response - If you have a health-related emergency or urgent issue, go to the nearest emergency room or call 911. If you are still in need of assistance, call us and we will immediately connect you with someone who will respond.

Prevention and early intervention - Once enrolled in our Partnership program, iCare will help keep you healthy and independent by working with public and private health and social service partners in the community. We can also provide you with information and intervention activities that prevent the risk of disability.

Self-Directed Supports

If you choose iCare you will have the option to choose Self-Directed Supports (SDS) as a way of receiving long-term care services. Choosing SDS means making your own decisions about how and from whom you receive your long-term care services. You take the lead in managing your care, having control over resources, including finances; and taking responsibility for personal decisions and actions. Your Team can tell you more about SDS if you are interested in this option.

iCare Family Care Partnership 2012 Disclaimers: The iCare Family Care Partnership (HMO SNP) is a Coordinated Care plan with a Medicare Advantage contract and a contract with the Wisconsin Medicaid program. You must use network plan providers except in emergent or urgent care situations. If your preferred provider is not in our plan, we will seek to include them in our network. If you obtain routine care from out-of-network providers, without first contacting your care team and getting prior authorization, neither Medicare, Wisconsin Medicaid or iCare Family Care Partnership (HMO SNP) will pay for these services. You must use network pharmacies to access your prescription drug benefit, except under non-routine circumstances when you cannot reasonably use network pharmacies. Generally, benefits are only available at iCare-contracted network pharmacies. Limitations, copayments, and restrictions may apply. Benefits, formulary, pharmacy network, premium and/or copayments/co-insurance may change on January 1, 2013. Please contact iCare for details. Premiums, co-pays, co-insurance, and deductibles may vary based on the level of Extra Help that you receive, please contact the plan for further details. This plan is available to anyone who has both Medical Assistance from the State and Medicare. The Resource Center can also assist you with information about eligibility and enrollment.
