Welcome to iCare Partnership!
Your child has a severe disability and will be leaving the school system soon. You’re concerned that you have to quit your job to take care of him. But then, how will the bills get paid?
iCare: We can provide a safety net so that you don’t need to quit your job to provide for your son or daughter. At age 18, your child may be eligible for Medicaid or Medicare or both. If your child is eligible for long term care benefits, they may elect to enroll in iCare’s Partnership program. A care team will be assigned to help with school-to-community transition. We can work together to develop a plan of care specific to the needs and preferences of both you and your child for supportive care.

What is iCare Partnership?
iCare Partnership is a managed care program that fully integrates Medicare and Medicaid services, and is designed to meet the long-term care (LTC) needs of people with physical disabilities, developmental disabilities and frail elderly adults.
If you have Medicare and become a member of iCare Partnership, you will have access to our Medicare Part D Prescription Drug benefit. If you do not have Medicare, your prescription drugs will be covered by iCare.
Member-Centered Care Teams
The Partnership care team offers you or your loved one a “circle of care.” A circle of care provides you with extensive support services. Our Partnership program offers a “whole-person” care model, where you are part of a team of caregivers with various skill-sets, including a nurse practitioner, registered nurse, social services coordinator, and an extensive network of other professionals. Your team is trained to assure you receive coordinated services to help you maintain your independence and remain active in the community.
As a member, you participate in team decision-making. You and your team work with your doctors and other health care providers to meet your medical and long-term care goals.
Your Partnership care team will conduct initial and yearly assessments to determine the best level of care for you. In the process, needs and goals will be discussed and you and your family can participate in preparing a life-style plan that includes self-management of conditions and behaviors. The care team will teach, model and prompt activities that promote healthier living.
The Partnership team at iCare will travel with you on your journey toward well being and happiness. We make a personal promise to meet your needs beyond typical long-term care and medical services. Our people empower you to reach your milestones and broaden your world, whether you need a ride to church, a nurse to go to the doctor with you, or more independence in your home. When choosing a Family Care Partnership program, you want iCare as your guide, advocate and traveling companion.

iCare Family Care Partnership 2012 Disclaimers: The iCare Family Care Partnership is a Coordinated Care plan with a Medicare Advantage contract and a contract with the Wisconsin Medicaid program. You must use network plan providers except in emergent or urgent care situations. If your preferred provider is not in our plan, we will seek to include them in our network. If you obtain routine care from out-of-network providers, without first contacting your care team and getting prior authorization, neither Medicare, Wisconsin Medicaid or iCare Family Care Partnership (HMO SNP) will pay for these services. You must use network pharmacies to access your prescription drug benefit, except under non-routine circumstances when you cannot reasonably use network pharmacies. Generally, benefits are only available at iCare-contracted network pharmacies. Limitations, copayments, and restrictions may apply. Benefits, formulary, pharmacy network, premium and/or copayments/co-insurance may change on January 1, 2013. Please contact iCare for details. Premiums, co-pays, co-insurance, and deductibles may vary based on the level of Extra Help that you receive, please contact the plan for further details. This plan is available to anyone who has both Medical Assistance from the State and Medicare.
