Organized in 1994,
iCare
is a Medicaid HMO
that manages access to health care and social services for people with
disabilities (physical, developmental and emotional) ages 19 and older,
who receive both Medicaid and Supplemental
Security Income (SSI) benefits. With over 8,000 members in our plan,
we are one of the largest Medicaid HMOs for those with
special needs in the State of Wisconsin.
With the iCare
program, we assist members with medical, dental, behavioral health, vision
and prescription drug coverage through care management. Care management recognizes medical,
behavioral and
social needs must be met in order to improve quality of life.

network provider, you have access to a variety of our resources to help you provide
quality care for your patients.
Our Philosophy
We incorporate a holistic approach to care, by focusing on treating the person
as a whole, not just
their disease state. Our care coordination model stratifies members
according to a specific risk level. The degree of intervention used
is based on this risk level classification that attempts to solve not only
specific medical issues but also behavioral health and social/life issues.
