About the Program

iCare contracts with myMTMcare to offer Medication Therapy Management (MTM) services to all eligible Independent Care Health Plan members in one of our Medicare plans. Through myMTMcare, specially trained Personal Pharmacists are trained to deliver these services by phone.

These pharmacists are qualified to help you get the best results from your medication while keeping out-of-pocket costs down. By using a myMTMcare Pharmacist, you get the extra attention you need to keep your medications on the right track.

All iCare members in one of our Medicare plans are eligible for MTM services at no additional cost.  Members may be contacted by phone to receive MTM services.

Members who meet the following criteria in 2024 will be eligible for MTM services and a Comprehensive Medication Review:

  1. Have 3 of the following chronic diseases: rheumatoid arthritis, chronic heart failure, diabetes, hypertension, depression, asthma, COPD, and cardiovascular disease
  2. Take 8 or more chronic medications
  3. Incurred one-fourth of the specified annual cost threshold ($5,330) in the previous three months

Some of the MTM Services That May Be Available To You Include:

Comprehensive Medication Review (CMR):
Your myMTMcare Pharmacist will contact you by phone to review all of your medications. This will help identify any duplications or conflicts, as well as help organize your medication schedule. The CMR service may take between 30 minutes to one hour. Following the CMR, you will receive a Medication Action Plan and Personal Medication List. You may want to bring those when you talk with your doctors or other health care providers.

Prescriber Consultation:
Your myMTMcare Pharmacist will talk with you and your doctor(s by phone, to resolve any problems found with your medications or offer suggestions to help you save money.

Medication Adherence Consultation:
Your myMTMcare pharmacist will talk to you about the importance of taking your chronic medications as prescribed, give suggestions on how to remember to take your medications as prescribed, and check in with you often to make sure you are continuing your medication adherence success.

Drug Information:
When starting a new medication, your myMTMcare Pharmacist will:

  • Talk to you about its purpose and correct use
  • Follow-up to make sure the drug is working right and you are not having any problems

Check if you are eligible. Simply call the Customer Service line at 1-800-777-4376 (TTY/TDD line at 1-800-947-3529). Eligible members can opt out of the MTM program, as participation is voluntary.

Although the MTM program is a special service offered at no cost to our Medicare members, it is not considered a benefit.
